Euglycemic Diabetic Ketoacidosis in a Patient with Type II Diabetes Mellitus, Ascites and Poor Nutrition on SGLT2 Inhibitor

چکیده
Title: This is the case of a patient with Euglycemic Diabetic Ketoacidosis (euDKA), ascites, poor nutrition and diabetes mellitus type II on SGLT2 inhibitor. Background: euDKA is an under-reported and under-diagnosed condition that mainly occurs in type I diabetes mellitus patients and in patients with prolonged starvation. The aim of this report is to shed the light on an additional risk factor for euDKA in patients with type I or type II diabetes: SGLT2 inhibitors. Methods and findings: We present the case of a 59 year-old man with type II diabetes on quadruple therapy including canagliflozin who presented for workup of ascites and anorexia and was found to have ketonemia and high anion gap acidosis with acceptable blood glucose levels. Patient’s acidosis and ketonemia did not improve except with the institution of intravenous insulin drip making the diagnosis of euDKA extremely plausible. Conclusion: In diabetic patients on SGLT2 inhibitors with poor oral intake, acidosis should raise a red flag for euDKA.

Euglycemic Diabetic Ketoacidosis in a 27 year-old female patient with type-1-Diabetes treated with sodium-glucose cotransporter-2 (SGLT2) inhibitor Canagliflozin
We are reporting a timely case of atypical euglycemic diabetic ketoacidosis in a type 1 diabetic patient treated with sodium-glucose cotransporter-2 (SGLT-2) inhibitor canagliflozin. The clinical history, physical examination findings and laboratory values are described. Other causes of acidosis such as salicylate toxicity or alcohol intoxication were excluded. Ketoacidosis resolved after incre...
متن کاملSafety of sodium-glucose co-transporter 2 inhibitors during Ramadan fasting: Evidence, perceptions and guidelines
Sodium-glucose co-transporter 2 (SGLT2) inhibitors are a new glucose-lowering therapy for T2DM with documented benefits on blood glucose, hypertension, weight reduction and long term cardiovascular benefit. They have an inherent osmotic diuretic effect and lead to some volume loss and possible dehydration. There is some concern about the safety of using SGLT2 inhibitors in Muslim type 2 diabete...
متن کاملEuglycemic diabetic ketoacidosis in type 2 diabetes treated with a sodium-glucose cotransporter-2 inhibitor.
More than 10 million Canadians are currently living with diabetes.1 Of those, 90% have type 2 diabetes mellitus (T2DM).1 Recently launched oral medications known as sodium-glucose cotransporter-2 (SGLT2) inhibitors were approved by the US Food and Drug Administration (FDA) in 2013 for treating T2DM.2 Approval by Health Canada was granted in 2014.3 Treatment with SGLT2 inhibitors (canagliflozin,...
متن کاملEuglycemic Diabetic Ketoacidosis with Persistent Diuresis Treated with Canagliflozin
Diabetic ketoacidosis is characterized by hyperglycemia, anion-gap acidosis, and increased plasma ketones. After the resolution of hyperglycemia, persistent diuresis is rare. We herein report the case of a 27-year-old Asian woman with type 2 diabetes who was treated with a sodium-glucose cotransporter 2 (SGLT2) inhibitor (canagliflozin) who developed euglycemic diabetic ketoacidosis and persist...
متن کاملEuglycemic Diabetic Ketoacidosis and Severe Acute Kidney Injury Secondary to off Label Use of Sodium Glucose Cotransporter-2 Inhibitor in a Type-1 Diabetic Patient.
Sodium glucose Cotransporter-2 (SGLT2) inhibitors are a new class of drug approved for the treatment of type-2 diabetes; however they are also increasingly used off label in type-1 diabetic patients. SGLT2 Inhibitors work by increasing glucose excretion in urine. Euglycemic diabetic ketoacidosis (DKA) is potentially life threatening side effect as patients have normal glucose and minimal sympto...
